Sahara Home Care is committed to offering comprehensive elderly in-home care services. As a part of our commitment to providing these services, we work with family members to determine client’s needs and match seniors with a homecare aide that has the right set of skills to meet those needs. Our homecare aides assist clients with non-medical services (e.g., grooming, bathing, toileting and dressing). We respect our clients’ privacy and all our clients’ personal, financial and health information is strictly confidential. Although the level of income does not affect eligibility for the program, an income level is established for each client to determine the client’s ability to contribute to the cost of care. The services are free to qualifying individuals. The essential in-home care services our agency provides includes personal care such as assistance with bathing and grooming as well dressing as well as cooking, cleaning, laundry,and running light errands Our Homecare Aides speak English and/or Urdu, Hindi, Gujrati, Punjabi, Sindhi, Bengali, Arabic, Malayalam, Pashto, Assyrian, Bosnian, Spanish, Tagalog andother South Asian Languages.
Seniors who qualify can receive free home care services. To be eligible, applicants must:

Be 60 years or older
Be a U.S. Citizen or legal alien
Be a resident of Illinois
Have non-exempt asset value of less than $17,500
Have an assessed need for care
Have applied for Medicaid
After potential clients submit an application, Care Coordination Units from their respective counties schedule an appointment to evaluate eligibility. Once approved, the CCU will draft a plan of care and assign the client a given number of hours.

Non-Seniors may also be eligible for the CCP Program if they have a disibility and meet the following requirements:

Be under the age of 60
Be at risk of imminent risk of nursing facility placement
Be a Medicaid recipient or have applied and obtained decision for it
Require services in the home costing the same or less than nursing facility costs
Be a resident of Illinois
Be a U.S. citizen or legal alien
Have a doctor’s approval of the initial plan of care
Score the required points in the Determination of Need (DON)
Have assets not exceeding the asset limit which is different for individuals under age 18 and those 18 and older
